Baker joins CareXM from Enhabit, one of the nation’s largest home health and hospice providers, where she served as Vice President of Care Management. With over 25 years of leadership across clinical operations, she has designed and scaled programs covering triage, virtual care, staff engagement, and patient access for tens of thousands of patients annually.
At CareXM, Baker will lead the Clinical Solutions group, which partners directly with providers to re-engineer clinical workflows, optimize labor models, and elevate patient experience.
