WellSky® Delivers the Technology and Expertise Hospitals Need to Thrive Under CMS’s New TEAM Model

With a vast nationwide network, advanced technology, and expert services, WellSky helps providers drive better outcomes, reduce costs, and navigate evolving value-based care models

WellSky, a leading global health and community care technology and services company, today announced enhanced capabilities to empower healthcare organizations to succeed in value-based care models, including the Centers for Medicare & Medicaid Services’ (CMS) new Transforming Episode Accountability Model (TEAM). As the discharge planning and referral management system of record for thousands of hospitals and post-acute providers, WellSky delivers the technology, data-driven insights, and deep industry expertise needed to optimize financial performance and streamline care coordination.

Launching Jan. 1, 2026, TEAM is a mandatory bundled payment model that holds acute care hospitals financially responsible for the cost and quality of care for Medicare beneficiaries undergoing inpatient surgical procedures. According to the Federal Register, TEAM is projected to save the Medicare program approximately $481 million over its five-year performance period (2026 through 2030).

Case managers, discharge planners, and care coordination teams will play a critical role in ensuring timely patient transitions, engaging post-acute providers, and closely monitoring patients throughout the 30-day post-discharge period to reduce readmissions and control costs. Failing to effectively manage these transitions can result in higher readmissions, increased episode costs, and missed performance targets, putting hospitals at financial risk under TEAM.

WellSky drives success in TEAM and value-based care with:

  • A nationwide network for seamless transitions

WellSky connects providers, payers, and community organizations through one of the largest post-discharge care networks in the country, spanning 2,500 hospitals, accountable care organizations (ACOs), and physician practices, and more than 130,000 post-discharge providers, facilitating over 54 million referrals and 17 million discharges each year. This extensive network helps hospitals participating in TEAM ensure patients are quickly connected to high-quality post-acute care.
  • Technology that powers value-based success

WellSky’s data-driven solutions provide predictive analytics, real-time insights, and interoperability to help organizations proactively manage episodes of care and succeed in value-based care models like TEAM.
    • Proactive, real-time data: Dynamically surface TEAM-eligible patients within the discharge planning solution, allowing providers to intervene earlier in the discharge process and avoid qualifying patients slipping through the cracks.
    • Cross-team communication: Collaborate with post-acute providers through messaging and chat to help reduce length of stay and lower costs.
    • High-performance post-acute network: Build a collaborative to proactively drive TEAM patients to higher-quality providers.
    • Comprehensive patient tracking: Alert care coordinators, emergency department case managers, and post-acute partners when a TEAM patient visits the emergency department or has a care transition event to redirect care when appropriate.
    • Optimize workflows: Assign tasks to care teams, such as engaging patient families early to set expectations and improve outcomes.
    • Provider performance insights: Monitor post-acute provider performance and view insights, including 30-day readmission rates for TEAM patients.
  • Comprehensive services for risk-based success
    • The WellSky Clinical Operations team provides onsite clinicians who work alongside providers to manage TEAM patients before and after procedures, extending their care to more patients across care settings. The WellSky Provider Engagement team takes a data-driven approach to build high-performing post-acute networks and educate acute case managers on TEAM workflows. This helps hospitals meet performance goals during the 30-day post-discharge period.
